( ) $10 PER DAY BOARDING FEE IF INSEMINATION IS SUCCESSFUL, OTHERWISE, IF      
     STUD FEE IS RETURNED, A $25 PER DAY BOARDING FEE WILL APPLY. 
( ) PICK PUPPY MALE OR FEMALE, TO BE DETERMINED BY OWNER OF STUD. (Includes Transportation to Atlantic City International Airport via commercial airlines or delivered at the bitch owners expense.)
The conditions of the contract are as follows:
1) It is understood that the OWNER of the STUD is not obligated to sign the application for registration of this litter until the stud fee has been paid for in full, or the pick puppy or puppies have been received. Initial here: __________
If no stud fee was paid, and puppy or puppies are to be picked in-lieu of payment, the following applies:
A) Stud dog owner will choose pick puppy or puppies by the 3rd day of life. (two pups will be picked:1st and 2nd choice. 2nd choice will be retained not for sale until 3 weeks of age.) In the case of a sudden death, or unforeseen problem with pick pup.
B) ALL sired puppies will retain and carry "Canonica’s” as its kennel name prefix.
This shall prefix all dogs’ names on the puppy applications.
3) (2) Puppies constitutes a litter. A repeat insemination will be granted back to the stud if, for any reason, less than (2) puppies survive at two days of age, 48 hours.
WE MAY allow a different female if this female becomes unbreedable, to be determined by owner of stud.
There will be NO refund of cash.
You will pay only $300 vet fee's to repeat the test and insemination procedure.

4) The OWNER of the BITCH is responsible for any and all veterinary and financial arrangements, including, but not limited to, traveling expenses, emergency vet care, and etc.

5) No insemination will take place until the OWNER of the BITCH has supplied to the OWNER of the STUD:
   1. A photocopy of BITCH'S IOEBA /CKC/BBKC Registration
   2. A three-generation pedigree or photos in advance if artificial by chilled semen. (Bitch           
       unseen).

3. Provide a Veterinary Certified Negative Brucellosis test, Fecal exam, Shot Records and to
       ensure that the said Bitch is free of parasites and flees prior to breeding.

7) The owner of Breeder Bitch hereby releases from responsibility Anthony Canonica and Michelle Krajewski and its household from any and all liabilities and/or damages to said bitch while in the care of Canonica’s Bullies for her breeding. Initial here: _________
8) By this contract it is specifically agreed that the stud dog owner is not obligated to sign the application for registration of this litter, until, and unless, the stud fee has been paid in full, or in case of a puppy or puppies (in-lieu of the stud fee) the stud dog owner has taken possession of the puppy or puppies as previously agreed.

9) In order to assure myself of the pregnancy of the bitch, the right to see and examine her is hereby granted, if so desired.
